This is a Mamiya 645 Pro 24MM f4 C Fisheye Lens. 

This lens is in excellent condition.  No haze, no fungus, and no separation.  Aperture is snappy and focus is smooth.  One of the leafs on the hood part has a slight ding but looks fine and and works fine.  Just needed to mention.  Comes with the front and rear caps.
